EASY HCFA 1500 Form Filler™ works
with Windows 95, 98, 2000, ME or NT and now Windows XP as well. It requires that you use Microsoft Word 97
or 2000 (Microsoft Office 97 or 2000), Office XP (2002) or 2003.
(Sorry, not
"Microsoft Works" unless it includes WORD or and not "WordPerfect" or "Microsoft
Office 95").
EASY HCFA 1500™ presents you with a series of on-screen fields. Each field
corresponds to a location on the HCFA 1500 form. You can type in your text or pick the text you want from a menu
list that drops down for you. On screen prompts advise you of the appropriate
content for each area. When you have completed the data entry for a
patient/client, save it with any name you like and then print it. When you
next do a claim for that patient, simply bring up the form data you entered
previously, change the dates of service and amounts (or anything else) and
print onto a blank HCFA 1500 form. This copy too can be saved to disk.
EASY HCFA 1500 ™ works as an add-in to Microsoft Word. It has no effect on
the speed with which Word operates. It requires minimal disk space
or memory. The program
includes an automatic uninstall routine (should you wish to remove it from Windows). Entries
are printed onto blank HCFA 1500 forms that you supply and feed through
your printer.
Your name, degree and tax ID number will be built into all programs. For NO additional charge, we offer to
customize your version of EASY HCFA 1500 Form Filler™ further so that
your address, phone, professional license
number, provider ID number(s) and "SIGNATURE
ON FILE" for boxes 12 and 13 print automatically. The program
will also allow you to enter the name and address of the insurance carrier at
the top to keep you organized and allow, if you wish, use of
window envelopes for easier mailing.
All of these features will save you time by automating
repetitive entries. And because blank HCFA 1500 forms are not all exactly identical, we test
print your copy on your HCFA forms before we send it to you, to make
sure all entries line up well.
Two Versions of the Program are available. Please review the info below to assure you purchase the one you need.Ordering the
wrong version will only delay your order.
INDIVIDUAL CUSTOM VERSION ($43.50) is sold to only to individuals or small group practices of 3 or fewer
practitioners.We require the individual practitioner's name, the Group Practice name, or the names of the
members of the Group (3 maximum) which will be permanently build into the first line of box 33. We will
personalize your version of EASY HCFA 1500 Form Filler™ so that your address, telephone, provider ID
number(s), professional license number and "on file" (for boxes 12 and 13) are printed automatically.
If you deal with multiple HMOs and have more than one provider number, each of those can be entered
on the form by pointing and clicking at the correct choice on a drop down menu. This version also includes
the ability to enter the insurance co. name and address at the top of the form allowing use of a window
envelope. Very fast, very easy and no typos or repetitive entries.
- Group Version ($98.50) has all of the flexible features of the Custom Version
but is designed for group practices of more than 3 practitioners, Clinics, Billing Services and businesses
such as Government Agencies, Hospitals, Medical Equipment Supply Companies, Health Service Agencies,
Fire (ambulance) Companies, Home Nursing Services, Schools, Home Health Care Agencies, Day Care Facilities,
Pharmacies, etc. Such organizations must purchase this version.

Licensing
Each of the above versions is sold licensed for installation on one computer. Organizations wishing to install the program on more than one machine or at more than one location must contact us regarding a multi site or Network Use license. (This does not apply to individual practitioners wanting to install the program on both their home and office machines).
